Community Links Social Prescribing

More support for patients throughout the county to improve their wellbeing is available thanks to this new project. In collaboration with 16 GP Practices in the county, hvoss secured money from NHS England to employ 3 Social Prescribers. The work is being trialled with a view to a full county roll out next year.

The Social Prescribing service is non medical, and works to connect people with services, information and advice that can improve their wellbeing. Social Prescribers also provide support and an important listening service to patients who are finding life a challenge.

The project is working closely with Herefordshire Council’s WISH and Community Brokers team, Public Health and Taurus Healthcare.

Social Prescribers are based in 16 GP Practices and take referrals from GP’s and health practitioners. Looking at non medical forms of support, advice and information Social Prescribers are able to support patients to improve their wellbeing. Some of the issues patients present with are low mood,  loneliness, housing, finance, staying independent at home.
